Document Requirements

These requirements are applicable for Non-US Citizens applying for a Dominican Republic Student visa in Washington DC

Please send TDS your current valid and signed passport. It must be valid for at least the duration of the requested visa or longer and must have at least one blank visa page. Foreign Embassies cannot issue a visa on a page not labeled as a Visa page. Amendment and Endorsement pages in the back of the passport cannot be used for issuance of a visa.
A visa cannot be stamped on a page even if only one Small Stamp is on the visa page. If your passport expires within 6 months, you must renew your passport and then we will secure your visa with the new passport.

Please send a copy of your flight itinerary. The itinerary must show the applicant's name, date of arrival and departure from the country, and flight information. If requesting a multiple entry visa your flight itinerary must show multiple entries into Thailand.
The Embassy does not recommend purchasing your ticket prior to a visa being issued.
A Photocopy of Permanent Resident Card (Green Card) or valid U.S. visa and I-94 is required.
Travelers who entered the USA after April 26th 2013 should print the electronic I-94 form from the
Department of Homeland Security.
Document with Proof of Scholarship (if applicable) or from the institution financially supporting the student, in the event that the studies are funded by an institution in their country of origin or a scholarship. This document must specify the amount of financial support or scholarship and its duration. In both cases this record must be written on official letterhead and certified by the institution.
Acceptance certification of the University or Center of Studies. This letter should specify the level of studies to be performed, the institution selected and the duration of the program.
Criminal Record Certificate issued by the appropriate authorities of the country in which the person resides at the time of the application. This document must be notarized and translated into Spanish and both the original and the translation Apostilled.

A Criminal Record Certificate is not required for minors.
Medical Certificate showing a diagnosis of the overall health of the applicant, indicating if he or she suffers from any contagious disease. It must be issued by the health authorities of the country in which the person resides at the time of the application. This document must be notarized, translated into Spanish and both the original and the translation Apostilled.
Documents showing the applicant's financial solvency (Letter of Bank, Letter of employment, titles of property). If a document is issued in the Dominican Republic, must be certified by a notary public and legalized by the Attorney General's Office (Procuradoría General de la República).
In the case of minors, permission of the parents or guardian when the applicant is not the mother or the father. If the child is not to be accompanied into the country by both parents, it is required that the parent who is not in the Dominican Republic authorizes the other parent to move with the child to the Dominican Republic. This document must be notarized, translated into Spanish and both the original and the translation Apostilled.
Visa application letter from the beneficiary addressed to the Consular Section containing name, nationality, place of residence and occupation.
