These requirements are applicable for US Citizens applying for a Dominican Republic Business visa in Washington DC

Please send TDS your current valid and signed passport. It must valid for at least the duration of the requested visa or longer and must have at least one blank visa page. Foreign Embassies cannot issue a visa on a page not labeled as a Visa page. Amendment and Endorsement pages in the back of the passport cannot be used for issuance of a visa.
A visa cannot be stamped on a page even if only one Small Stamp is on the visa page. If your passport expires within 6 months, you must renew your passport and then we will secure your visa with the new passport.

Please send a copy of your flight itinerary. The itinerary must show the applicant's name, date of arrival and departure from the country, and flight information.
The Embassy does not recommend purchasing your ticket prior to a visa being issued.

Business LetterBusiness Letter

A business letter of responsibility (sample) is required, to be typewritten on your company letterhead and addressed to:
Embassy of Dominican Republic, Visa Section, Washington DC
The letter must explain all of the following:
Nature of business to be performed
Name and address of reference to be visited
This letter should request Multiple Entries visa if required
Guarantee of return transportation and sufficient funds for the visit
Applicants passport number
The letter should be signed by an officer of the company other than the applicant.

Hotel ConfirmationHotel Confirmation

You must provide a copy of your hotel confirmation, the confirmation must confirm your stay while in the Dominican Republic.

Financial ResponsibilityFinancial Responsibility

Please provide 3 most recent bank statements or proof of sufficient funds to be used during your travels. Applicant's name should appear on statements.

Criminal Record CertificateCriminal Record Certificate

Must be issued by the proper authorities of the country in which the person resides at the time of application. If the applicant has previously resided in the Dominican Republic that document must be issued by the Attorney General’s Office. This document must be notarized, translated into Spanish and both the original document and translation must be apostilled. Once the original document is apostilled, it can be sent for translation at the Consulate.
