REQUIREMENTS
TO EXPEDITE U.S. PASSPORTS
NEW OR FIRST TIME ISSUE
You are considered a new or first time issue
applicant if you answer YES to any of the following:
You are
applying for a US passport for the first time.
Your expired US passport is not in
your possesion.
Your previous US passport has expired and was issued
more than 15 years ago.
Your
previous US passport was issued when you were under 16
years.
Instructions and Document Completion:
1.
Completed application form DS-11.
- Applicants requesting expedited passport service through TDS, MUST now use an online version of the form DS-11, rather than handwriting the required personal data.
- DO NOT SIGN the DS-11 form until asked to do so by the Post Office or Court House passport clerk.
- PASSPORT CARDS: The new Passport Cards cannot be expedited at this time and take approximately 4-5 weeks to issue. If you are applying for a regular passport only, please mark the box on your passport application next to "Passport Book". If you also want to apply for a Passport Card you should also mark the box next "Passport Card". There is an additional $20.00 government fee for the Passport Card and the Passport Card will be processed separately from your regular passport (in about 4-5 weeks) then mailed to you directly by First Class mail.
- If replacing a lost passport form DSP-64 must also be completed.

You MUST print and submit TWO copies of your completed application DS-11. One copy for the sealed envelope and the other copy for inclusion in the FEDEX Envelope, paper-clipped to the outside of your sealed envelope (see #9 below).

Letter of Authorization.

- You MUST print, then complete and include TWO COPIES of a Letter "Attn: U.S Passport Agency" stating: "I, (applicant name), hereby authorize Travel Document Systems to pick up my passport, and to discuss the status of my application with the Passport Services staff."
- One copy of this letter, with original signature in BLUE INK, should be placed inside your sealed envelope by the passport clerk and the other should be paper-clipped to the outside of that envelope
4.
Proof of U.S. Citizenship:
- A certified birth certificate, issued by state or county health department or vital statistics bureau (not hospital certificates); -or-
- A naturalization certificate; -or-
- A certificate of citizenship; -or-
- A Consular report of birth abroad; -or-
- A previous U.S. passport (if issued more than 15 years ago, or you were under 16 years when your most recent passport was issued).
5.
Proof of Identity:
- A Valid Drivers Licence issued more than 6 months ago;
- If the above-mentioned Driver's License isn't available, then you need to supply an alternate form of identity.
6.
Appointment:
If you require an emergency passport issued in 1-3 business days, you must have an appointment to proceed. Please call 1-800-874-5100 for an appointment. TDS must receive your document at least one business day prior to your appointment.

- All minor children applying for passports are required to appear in person at the local acceptance agency.
9.
Execution of your Passport Application:
- Take the above requirements to your local acceptance agent for Passport
Applications (usually a post office or county clerk or court house).
- Pay the clerk the U.S. government fee, including rush fees, and execution fees. Be prepared to write two checks for the total amount, as the clerk will retain $25.00 execution fee.
- $135.00 + $25.00 = $160.00 for adults,
- $120.00 + $25.00 = $145.00 for children under 16 years.
Please ask the clerk NOT to include the cost of USPS return postage in your payment of passport fees.
Please write your name and date of birth on the check.
Credit cards are not accepted for this part of the transaction.
Ask the clerk to execute the passport application documents and place them in a sealed envelope addressed to "U.S. Passport Agency".
You must not open this sealed envelope again before shipping it.
The sealed envelope is considered "expired" by the Passport Agency after 5 business days. Please please send your application to TDS the same day it is signed.
10.
SUMMARY:
Please be sure to make separate photocopies of:
- Your Application Form DS-11
- Your Letter of Authorization
- Your Proof of Departure (Itinerary)
- Your Birth Certificate or Proof of Citizenship
The following items must be placed inside the sealed envelope executed by the Passport Clerk at the Post Office or County Court, then all documents should be placed inside a FedEx envelop for shipping to Travel Document Systems, Inc.
- Your Proof of U.S. Citizenship
- The Application Form DS-11, the photographs and the appropriate Government fees
- The Letter of Authorization
- The Proof of Departure (Itinerary)
The following items must be attached (paper-clip) to the outside of the sealed envelope.
- The second copy of the Application Form DS-11
- The second copy of the Letter of Authorization
- The photocopy of the Proof of Departure (Itinerary)
- The photocopy of your Birth Certificate or Proof of Citizenship
- A completed Cover Page and fees/credit card authorization for TDS expediting services

URGENT
APPLICATIONS SHOULD BE SHIPPED TO TDS USING ONLY THE FEDEX COMPANY

Your
application will be hand carried to the Passport Agency in
Washington, D.C.. There, the Passport Agent will examine the
application and if all documents are in order, assign a date
the passport will be issued, according to departure date from
the U.S.

Urgent
issue applications (which may be issued between one and three business days) require proof of departure within seven business
days of being submitted to the passport agency and MUST be
shipped to TDS using only the FEDEX Priority Overnight service. If requesting 1-3 days service please call a TDS representative 1-(800)874-5100, before shipping, to ensure availability of this service. .

Other urgent applications can be issued in 7 business
days, less urgent applications are usually issued within 10+ business days.

Passport
applications submitted by mail to passport agencies routinely
take 6 to 8 weeks to be processed. TDS expedites applications
by hand carrying them to the U.S. Passport Agency, and following
up on the progress. Please note that TDS is not in a position
to be able to offer any guarantees of passports to be issued
by a specific date. Issue is determined by the urgency of
your departure, the acceptance of your appplication without
addidtional information being required, and passport agency
workloads, particularly during peak travel periods.

A
48-page business travelers passport can be issued in lieu
of the usual 24-page passport on request at no additional
charge (however, ONLY if the larger books are available at
the time of issue). Submit a written request on your letterhead applying for a 48-page passport.
