VISA APPLICATION FORMS
This Visa Application is on two
pages of white paper measuring 8 1/2 by 14 (LEGAL SIZE) which
must be photocopied back-to-back on a single sheet. It should
not be enlarged or reduced. This application form should only
be used at the Washington embassy.

the Acrobat Reader version that is suitable to your system from
the Adobe web site.
