Travel
- Submit
your U.S. Official or Diplomatic Passport, must have at least
6 months remaining validity
- 3 Visa
Application Forms (2MB) fully completed
and signed
- 4 Passport-type
photographs required
- One Report of
Arrival Form (attached to the Application Form)
- One Completed
Work History Form (attached to the Application Form)
- Completed
Cover Page and Credit Card Authorization forms (print from browser)
- Letter
from sending department or agency, addressed to:
Embassy of Myanmar, Visa Section, Washington D.C.
The letter must include all of the the following information:
- Applicant's
position and title
- Purpose
of travel

Contact person in Myanmar
