Required to Submit

• U.S. Passport (not a copy), must be valid for at least 6 months from date of entry.
• 2 Brazil Visa Application Forms, fully completed and signed
• 2 Professional Passport-type photographs (digital potographs not accepted)
• Completed Cover Page and Credit Card Authorization forms (print from browser)
• Copy of round trip ticket, or itinerary from an air carrier or travel agent attesting to the acquisition of the ticket, showing flight information and dates of departure to and from Brazil.
• Proof of residence in the Consular Jurisdiction for the past 12 months.
• Recent Non-Criminal record issued by the local Police Department.
• A business letter from your firm, addressed to: "Consulate General of Brazil, Visa Section, San Francisco, CA", explaining the purpose of the trip in detail and the nature of the duties to be performed in detail, stating name and full address of company to be visited in Brazil, guaranteeing financial support for the applicant while in Brazil, and return transportation. This letter must be signed by an officer of the company other than the applicant. THE SIGNATURE MUST BE NOTARIZED.
