• 1 Visa Application Form, completed and signed
• 1 Recent Passport-Type Photograph
• Proof of sufficient funds for stay in Bolivia
• Completed Cover Page and Credit Card Authorization forms (print from browser)
• Copy of itinerary showing round trip travel
• International Certificate of Vaccination for Yellow Fever
• Copy of U.S. Alien Registration Card (Green Card) or proof of visa status in USA

• A business letter of responsibility is required, to be to be typewritten on your company letterhead and addressed to:
  
  Consulate General of Bolivia, Visa Section, New York, N.Y
  
  The letter must explain all of the following:
• Purpose of the trip
• Name and address of company to be visited
• Guarantee of financial responsibility and return transportation
